1. Six weeks prior to your scheduled event, download the Radio Tag Information Form.
2. Fill it in (print it out and hand-write, or fill out on your computer).
3. Send it in (use the PO Box address on the form, or the email listed below it).
4. We will reply to the contact person listed on the form confirming broadcast dates.
